- A 2' x 2' identification photograph that was taken in the past 12 months

- The completed 'Census Questionnaire'
- A verification form proving that he or she is currently licensed in another US state
- A satisfactory nursing employment reference from the 3 year period preceding the application
- Proof that he or she is an Idaho resident
- A completed and notarized affidavit from page 2 of the application
- A fingerprint card (including the $42.00 processing fee)
- A completed 'NonCriminal Justice Applicant Privacy Statement'
- The licensure fee with an additional $25.00 temporary permit fee
- Proof that he or she is currently licensed in good standing in another US state
- The licensure fee with an additional $25.00 temporary permit fee
- Proof that he or she was previously licensed
- An application for licensure form, completed in ink and notarized by a notary public
- A 2' x 2' photograph of his or her head and shoulders
- The licensure fee of $100.00
- The 'Census Questionnaire' form
- An affidavit of graduation that is sent directly from his or her nursing program to the Board of Nursing
- A fingerprint card (including the $42.00 processing fee)
- A completed 'NonCriminal Justice Applicant Privacy Statement'
- A completed, signed, and notarized application
- A 2' x 2' photograph of his or her head and shoulders taken within the past 12 months
- A personal check for the $125.00 reinstatement fee
- The 'Census Questionnaire' form
- A satisfactory nursing employment reference from the 3 year period preceding the application
- A fingerprint card (including the $42.00 processing fee)
- A completed 'NonCriminal Justice Applicant Privacy Statement'
