In order to become an RN by endorsement in Alaska, an individual must submit:

  • A completed and notarized application
  • A 2" x 2" passport sized photograph of his or her face taken in the 6 months preceding the application
  • A check or money order for $284.00
  • A completed fingerprint card
  • Proof of licensure sent directly from the state in which the individual is currently licensed
  • Verification that the individual has worked at least 320 hours during the 2 years preceding the application
