Although doctors are responsible for most of the important decisions and treatment plans, licensed practical nurses help patients to feel comfortable and assist with much of the day-to-day care.Becoming an LPN in Utah
The first step in becoming an LPN in Utah is to complete an education program that is accredited by the Commission on Collegiate Nursing Education, National League for Nursing Accrediting Commission, or Council on Accreditation of Nurse Anesthesia Educational Programs. The following items must be attached with a licensure application:- Division of Occupational Licensing (DOPL) application
- $100 application fee
- 2 fingerprint cards
- New graduates: official transcript
- Those applying for licensure by endorsement: verification of licensure
- Graduates of foreign nursing schools who have never been licensed: Commission on Graduates of Foreign Nursing Schools (CGFNS) certificate
- Those applying for LPN equivalency: letter of equivalency from current RN program
- NCLEX registration (if applicable)
Renewing an LPN License
LPN license renewal in Utah is based on a set date, rather than date of license. All licenses expire on January 31st of even-numbered years. The first renewal period may be greater or less than 2 years, depending on how far into the 2 year renewal period the license was obtained.Additional Resources
