Licensure by Examination in West Virginia

In order to become an RN by examination in West Virginia, an individual must submit:

  • A completed board licensure application
  • A completed test service application
  • Criminal background check information
  • A money order or cashier's check for $51.50 to cover the application fee

Licensure by Endorsement in West Virginia

In order to become an RN by endorsement in West Virginia, an individual must submit:

  • A completed endorsement application that is signed and notarized
  • A money or order or cashier's check for $60.00 (plus an additional $10.00 if he or she is seeking a temporary permit)
  • A 2' x 2' passport photo of his or her head and shoulders that is signed by the individual

The top part of the WV Verification Form must be completed and mailed to the state where the individual was originally licensed.

Temporary Permits and Renewal

Temporary permits are issued for a period of 90 days to applicants who are determined to be eligible. Repeat examination applicants, graduates of foreign nursing programs, and veteran applicants are not eligible to receive a temporary permit. A temporary permit becomes invalid if an applicant fails the licensure examination.

All licenses expire annually on October 31.
