In order to become an LPN in Arizona, an individual must have:

  • Received a diploma or certificate from a state-approved nursing program
  • Passed the National Council Licensure Examination (NCLEX)
  • Earned a previous or current license in another state or territory
  • Met one of the following requirements:
    • Practiced as a nurse for 960 hours or more in the past 5 years
    • Graduated from a nursing program and obtained a degree within the past 5 years
    • Completed an Arizona Board approved refresher course in the past 5 years
    • Obtained an advanced nursing degree in the past 5 years

If an applicant has been trained as an LPN in a foreign country, he or she must submit:

  • Proof that he or she has met the educational requirements
  • Proof of English Language Skills
  • Proof of Practice
  • Proof that he or she has passed the NCLEX
